Study limitations

By nature of the study design, the survey respondents were limited to those who use social media. Furthermore, advertisements were targeted at those who reported interests in “parenting,” “pediatrics,” “urology,” and “urinary tract” thus potentially created selection bias. That is, participants may have had a unique experience with UTI leading them to invest time to complete this survey. Additionally, the population of individuals who answered this survey on Facebook is not representative of the entire population with > 80% of participants identifying as White and female. CONCLUSION

Evidence regarding the manipulation of gut microbiota composition as an effective cure for IBS is increasing and, to date, probiotic supplementation and antimicrobial therapy with not absorbable antibiotics are promising treatment. However, all meta-analysis point out to the weakness of the majority of the studies and recommend additional RCT trials to confirm the positive findings reported by small studies. Specifically, additional information on type of probiotic, doses, side effects and time of administration are required, as well as data on patient subtypes. Prebiotics are often burdened by unwanted side effects and synbiotics lack of sufficient numbers of clinical trials on their efficacy and safeness. FMT might be a reasonable option for treating IBS, as it is an inexpensive and easy treatment, but standardized controlled trials are necessary to ascertain which patients are eligible, the most effective regiment as well as the most acceptable method of administration of the donor’s microbiota. For these therapeutic options, a careful selection of patients, a close monitoring of clinical data and side effects and a long-term follow-up are necessary, as well as more information on modification of host microbiota composition.

Conclusion

The results of this study provide novel evidence that zinc ascorbate inhibits the growth of P. acnes, and its concentration (0.064%) is sufficiently lower than the normal dose (5%) of other ascorbic acid derivatives. In addition, zinc ascorbate shows an additive antimicrobial effect in vitro in combination with clindamycin, erythromycin, and chloramphenicol against clindamycin-sensitive and/or clindamycin-resistant P. acnes. These findings may provide novel insights into acne therapy. Recently, combination therapy with benzoyl peroxide and clindamycin has become common worldwide. Further experiments are needed to compare zinc ascorbate plus clindamycin and benzyol peroxide plus clindamycin.

DISCUSSION

P. aeruginosa still remains an important opportunistic cause of nosocomial infection and has developed resistance to various antimicrobial agents in burn centers (1, 5). This study was carried out for the first time at a single referral burn center in the Guilan province. The results of this study showed that 86 (47%) isolates contained P. aeruginosa that is in agreement with the results of previous studies (13, 14). The results of some studies demonstrated a higher frequency of P. aeruginosa among the patients with burn infections especially in many developing countries such as India 59% (15), Turkey 57% (16) and Pakistan 54.4% (17). In Iran, the highest frequency rate was reported as 73%and 70.5% in two old referral burn centers in Tehran, Iran (18, 19). In another study, however, a lower frequency rate of P. aeruginosa was reported from a burn hospital in Ahvaz, Iran (20). Resistance to antimicrobial agents is the main problem among P. aeruginosa strains isolated from wound infections in burn centers.

بیماری اسهال ناشی از نژاد های اسهال زای اشرشیاکلی یکی از مشکلات سلامتی در سراسر جهان است، به ویژه در کشورهای در حال توسعه. همچنین ادامه شیوع هزینه های سلامتی را افزایش می دهد. بیماری های اسهالی به عنوان چهارمین عامل مرگ و میر در جهان هستند، و مورد توجه قرار گرفته اند که بیماری های عفونی سبب 9.2 میلیون مرگ در کشورهای در حال توسعه می شود. نژادهای اشرشیاکلی اسهال زای مقاوم به آنتی بیوتیک معمولا وابسته به آنتی بیوتیک های بتا-لاکتام هستند. برای استفاده موثر از آنتی بیوتیک ها در مدیریت و درمان بالینی، نظارت پیوسته از مقاومت آنتی میکروبی مطلقا ضروری است. استعمال آنتی بیوتیک و ناقل های بیمارستانی به ویژه اوکسی ایمینوسفالوسپورین ها، به خوبی عوامل خطر را برای توزیع باکتریهای تولید کننده ESBLs ثابت نمود. این مطالعه اطلاعاتی را در رابطه با مساله مقاومت به انتی بیوتیک در نژادهای اسهال زای اشرشیاکلی ایزوله شده از بیماران وارد شده در کیلینک ها، بیمارستان ها و بیماران سرپایی آماده نمود. نتایج نشان داد که این نژادهای باکتری اشرشیاکلی میزان مقاومت به آنتی بیوتیک های تجویز شده بالایی را نشان دادند. مقاومت در مقابل پنی سیلین و بازدارنده های فولات خیلی بالا بود. مقاومت به آمپی سیلین در میان نژادهای اشرشیاکلی 36.11 درصد بود که پایین تر از مطالع دیگر در ایران بود. مقاومت به آمپی سیلین در میان باکتری های اشرشیاکلی اسهال زا احتمالا ناشی از استفاده پیوسته از آن برای سال های زیاد است. مقاومت به ایمی پنم 5.56 درصد مشخص شد. ایمی پنم یک آنتی بیوتیک کارباپنم است و فعالیت بالایی در مقابل Entrobactraceae  تولید کننده ESBLs دارد. این دارو مقاومت بالایی به بتا-لاکتاماز دارد و دارای یک ویژگی غیر معمول است. آن سبب یک اثر پس آنتی بیوتیکی بر باکتریایی گرم منفی می شود. مقاومت به جنتامایسین 8.33 درصد مشخص شد که در مقایسه با مطالعه گزارش شدهه توسط اصلانی و همکارانش پایین است. اما در مطالعه ما، 5.56 درصد مقاومت به آمیکاسین یافتیم و مشخص شد که آن اثر بیشتری نسبت به آمینوگلوکوزید در مقابل اشرشیاکلی اسهال زا دارد. میزان مقاومت در مقابل آمینوگلوکوزیدها یعنی جنتامایسین و آمیکاسین پایین بود، اما مساوی با کارباپنم، نیتروفورانتوئین و فلوروکوئینولون بود. مقاومت به سیپروفلوکزاسین در مطالعه ما 8.33 درصد بود، که موافق با گزارش دیگر در ایران بود. این مقاومت ممکن است ناشی از استفاده از فلوروکوئینولون ها به عنوان داروی انتخابی در عفونت های اداری باشد. میزان مقاومت گزارش شده در این مطالعه موافق با برخی گزارشات از ویتنامريال نیجریه، ایران و برزیل بود، اما پایین تر از اسپانیا و آمریکا، تایلند و پاکستان بود. عوامل بسیاری ممکن است در افزایش میزان مقاومت به آنتی بیوتیک تاثیر داشته باشد شامل: کاربرد نادرست آنتی بیوتیک ها در مراقبت های حرفه ای از سلامتی یا افراد غیر حرفه ای، کاربرد بد آنتی بیوتیک ها برای استفاده عمومی (آنتی بیوتیک ها می توانند در برخی نواحی بدن تجویز دارویی دکتر استفاده شود)، و نظارت ناکافی ناشی از فقدان اطلاعات از آزمون حساسیت ضد میکروبی روزمره، همچون گزارش از کشورهای در حال توسعه دیگر باشد.

Diarrhea is one of the world᾽s widespread health problems, with more than two million deaths in each year. Diarrheal disease is caused by a range of enteric pathogens such as viruses, bacteria, and parasites. Diarrheagenic Escherichia coli (DEC) and Shigella spp. are the most popular bacteria causing diarrhea (1). Emergence and dispersion of antibiotic resistance is well documented in bacterial isolates worldwide, particularly in developing countries (2, 3). Antibiotic resistance prevalence in E. coli is an effective marker for antibiotic resistance in each community (2, 4). E. coli is famous to be efficiently capable of accepting and transferring genetic materials and, under stress, readily transfers those genetic materials to enteric pathogens including Salmonella, Yersinia, Vibrio, and Shigella species. Therefore, it is considered an important reservoir of transferable antibiotic resistance (2-4). There is a global interest regarding advent and the rise of resistance to generally used antibiotics in bacteria (5). Irregular usage of antibiotic is possibly more important factor promoting the emergence, selection and distribution of antibiotic-resistant bacteria in human and veterinary medicine (6). This global usage of antibiotics could be connected with the selection of antibacterial resistance mechanisms in both nonpathogenic and pathogenic strains of E. coli. β-lactams are one of the most widely used antibiotics in both human and veterinary medicine (4). β-lactam resistance in Enterobacteriaceae is primarily conducted by β-lactamases. However a range of β-lactamases have been characterized, typically TEM, CTX-M and SHV enzymes are those prevalently observed among Enterobacteriaceae and have been increasingly found throughout the world (7, 8). Mutations in the genes encoding these enzymes can increase the spectrum of the activity of enzyme to include penicillins, the extended-spectrum cephalosporins and aztreonam. These enzymes are named extended-spectrum β-lactamases (ESBLs) (8). Antimicrobials resistant bacteria is a global problem (9). Routine monitoring of antibiotic resistance is important in order to prepare data for antibiotic therapy and resistance control (2). Also, understanding the molecular foundations of resistance can play an important role in the progress of new strategies to fight against this phenomenon (9). It is mandatory to clearly identifying the hazardous factors for infections owing to ESBLs-producing bacteria, therefor efficient strategies to decrease spread of these infection agents can be developed. Various studies have been done in an attempt to identify hazardous factors for infections owing to ESBLs-producer bacteria, but the results have been largely different. However these differences could be due in part to real disparity in the epidemiology of various outbreaks (10). The aim of the present study was to determine the types of β-lactamases and antimicrobial susceptibility pattern of DEC strains recovered from diarrheal specimens of adult patients with diarrhea in the south of Iran. Experimental Samples collection During the period from March, 2010 to December 2010, a total of 54 DEC were collected from patients with diarrhea admitted to all of the university hospitals in Shiraz. Written informed consent was obtained from all patients. All steps of this study were approved by the Ethical Committee of Islamic Azad University. Initial isolation of samples was performed on MacConkey, EMB and VRBA and confirmed by using standard biochemical tests, including indole, TSI, citrate, urea, MR, VP, LD, OD and SIM tests (Merck, Germany). Phenotypic detection of ESBLs production A modified version of the Jarlier doubledisk synergy (DDS) method (11) for detecting calvulanic acid (CLA) synergy was used. Ceftazidime (30 µg) and cefotaxime (5 µg) disks (Oxoid), were placed around an amoxicillin (20 µg)-clavulanic acid (10 µg) disk at a distance of 25 to 30 mm from center to center. A clearly visible extension of the edge of the inhibition zone of any disk towards the amoxicillinclavulanic acid disk was interpreted as positive for CLA synergy (11). Antimicrobial susceptibility testing Isolates were subjected to standard disc diffusion testing in accordance with the recommendation of the Clinical Laboratory Standard Institute (12). Isolates were tested for resistance to the following antibiotics: cefotaxime (30 μg), ceftriaxone (30 μg), cefexime (5 μg), ampicillin (10 μg), penicillin (10 U), imipenem (10 μg), ciprofloxacin (5 μg), levofloxacin (5 μg), chloramphenicol (30 μg), tetracycline (30 μg), sulfamethoxazole– trimethpprim (1.25 and 23.75 μg), gentamicin (10 μg), amikacin (30 μg) and nitrofurantoin (300 μg) (Mast Diagnostics, Merseyside, UK). Quality control was performed as recommended using the E. coli strain ATCC 25922. DNA extraction E. coli isolates were grown overnight in 5 ml of Luria Bertani (LB) broth (Merck) at 37°C. One milliliter of cell suspension for each isolate was transferred to 1.5-ml tubes and centrifuged at 13000 rpm for 5 min.

Evolution of antibiotic resistance in MDR enterococci The advent of the antibiotic era spurred a new revolution in modern medicine. With the discovery of antimicrobial agents and the understanding of the microbiological basis of disease, infection became a treatable disease with remarkable results. However, clinicians rapidly realized that certain microorganisms appear to respond less well to specific antimicrobial agents. For example, with the introduction of penicillin, streptococcal infection was successfully treated with this agent. However, a subset of ‘streptococcal’ organisms (later known as enterococci) appeared to respond less well to penicillin due to an inherent tolerance to the killing action of these compounds [3]. It was later found that the addition of aminoglycosides (streptomycin was discovered in 1944 [4]) to penicillin produced synergistic activity improving the cure rates for enterococcal infective endocarditis from 40 to 88% [5]. This synergistic effect was seen despite the fact that enterococci are also inherently less susceptible to aminoglycosides compared to other gram-positive bacteria. Thus, the combination of a cell-wall active agent (i.e., ampicillin/penicillin) plus an aminoglycoside became the standard of care for deep-seated enterococcal infections and this combination is still used to the present day [6]. Though it was not known at that time, the seeds of the modern MDR enterococci were already being sown. Using comparative genomics, Lebreton et al. demonstrated that the modern-day MDR Enterococcus faecium belongs to a genetic clade that appears to have diverged from animal-adapted E. faecium isolates about 75 years ago, coinciding with the introduction of antibiotics in clinical practice [7]. Features of this clade (designated A1) include an increase in mobile genetic elements, alterations in metabolism and hypermutability, a skill set that provides E. faecium a malleable genome in the face of multiple selective pressures. The remarkable increase in the use of antimicrobials in clinical medicine in the latter half of the 20th century provided the selective environment for these microorganisms to evolve by recruiting a variety of antibiotic resistance determinants. Among the most distinct examples of this adaptability is the acquisition of the genes encoding vancomycin resistance. Van-comycin use was associated with the emergence and spread of methicillin-resistant Staphylococcus aureus (MRSA) in the 1960s. However, unlike MRSA, enterococci have been able to recruit and maintain a variety of gene clusters encoding the biochemical machinery for vancomycin resistance. Of concern, E. faecium is also increasingly identified in nosocomial infections, now occurring as frequently as isolates of Enterococcus faecalis (whose rates of vancomycin resistance are about 5%) [2]. More recently, enterococci have also served as donors of vancomycin resistance gene clusters to more pathogenic microorganisms such as MRSA, an event considered to be a serious public health threat [9,10]. Despite the availability of anti-gram-positive agents (e.g., line-zolid, quinupristin/ dalfopristin [Q/D], daptomycin [DAP], tige-cycline), enterococci have rapidly adapted and emergence of resistance to all these newer agents have been well documented. This phenomenon makes the treatment of MDR enterococcal infections a daunting clinical challenge. Thus, in the following sections, we will review the molecular mechanisms of antibiotic resistance in enterococci with the objective to place such mechanisms in context within a clinical perspective and explore innovative strategies to combat these recalcitrant microorganisms.
