مقدمه

از زمان مقدمه مقایسه های سینه های مسدود در ۱۹۶۰{۱} ، احیای قلبی – ریوی یکی از استاندارهای دخیل در CPA به شمار می رود . دنبال کردن به مدت پنج دهه باعث مزیت هایی در تکنیک های احیا شامل توسعه پروسه های حمایت از سلامت قلب و ریه (ACLS) و مرکز postarrest  گردیده است {۲}. اما نسبت های بهبود یافته به ترخیص شده به میزان ۱۵ تا ۲۰ درصد می باشد که اندکی تغییر کرده است و بهبود یافتگان با کیفیت حیات بالا نادر می باشد. 

با فراهم آوردن اطلاعات لازم در خصوص میزان پائین موفقیت و هزینه بالای ACLS در بیمارستان و درک دقیق پیش بینی های بهبود یافتن از CPA می تواند تصمیم گیری برای بیماران ، خانواده ها و احیا کنندگان مرکز درمانی و پرداخت کنندگان را زمانی که کاربردهای مناسب تلاش های احیا و بهوش آوری را لحاظ می کنند ، بهبود ببخشد.

اخیرا استفاده از NRCPR ( دفتر اسناد ملی احیای قلبی ریوی ) برای گزارش نتایج و  ارزیابی فاکتورهای ریسک یک جمعیت بزرگ بیماران که می توان تحقیقات ساده ای روی آنها انجام داد افزایش یافته است . ما علاقه مندیم تعیین کنیم که آیا فاکتورهای تاثیرگذار روی نتایج CPA در مرکز درمانی آموزش شهری با این ویژگی که intesivist بیماران را در طول دوره قبل از arrest و بلافاصله بعد از آن مدیریت می کنند مشابه می باشد. ما یک مرور بازنگرانه از همه تلاش های انجام یافته برای احیا و بهوش آوری در UMMC از سال ۲۰۰۰ تا ۲۰۰۵ انجام داده و آمارهای مربوط به بیماران و ویژگی های arrest  نوعی بخش و سرویس ، و حالت های مشاهده شده  در بهبودیافتگان را مورد آزمایش قرار می دهیم . فرض کرده ایم که تفاوت های مهمی در نتایج و فاکتورهای ریسک از موسسه ها با جمعیت آماری بیماران متفاوت و CPA و مدیریت  post-CPA متفاوت با نتایج و داده های موجود در ادبیات پزشکی وجود دارد . هرگونه تفاوت چشمگیر می توانند بطور بحرانی در تصمیم گیری با در نظر گرفتن تلاش های احیا و بهوش آوری در موسسات که جمعیت بیماران آنها با ارقام پایگاه های اطلاعاتی NRCPR متفاوت می باشد ، موثر باشد.

۳٫ نتایج
در طول دوره تحقیق ۷۲۹ (arrest ) در ۷۰۹ بیمار رخ داده جدول یک جزئیات آمارهای بیمار ، سرویس پذیرش و نوع تخت را برای بیماران بیان می کند . ۴۷۶ (۶۷%) مذکر بودند با میانگین سن ۱۶+۵۸ سال . مقایسه های امریکایی – افریقایی و سفید پوست، که بترتیب ۲۹۰(۴۳%) و ۳۴۹(۸/۵۱%)بود انجام شد اغلب بیماران سرویس پذیرش درمانی یا جراحی و ۱۲۲(۷/۱۶%)در قلب شناسی بودند اکثریت arrest ،(۱/۶۲% ۳۵۳ )در IV  رخ داد.

ویژگی های arrest و نتایج احیا در جدول ح نشان داده می شوند نتوسط طول دوره cpa 23±۲۴ دقیقه می باشد و cpa بصورت هموار درطول روز پخش شده اند (۸/۵۰%) ۳۷۷ از cpa ها قلبی (۹/۲۴%) ۱۸۵ تنفسی  و (۳/۲۴%) ۱۸۰ نوع از arrest قابل تعییمن نبود اغلب arrest ها دارای شاهد بودند (و ۶۴ مورد ۴/۹۱%) و به صورت مانیتورینگ دیده شده میزان (۲/۸۴%)بودند. (۹/۵۳%)۳۸۴ از بیماران یک بازگشت اولیه به ئنبال تلاش های احیا را دارند(۳/۲۱%)۱۵۰ بهبود یافته و مرخص شدند و (۶/۹%)۷۰ به خانه مرخص شدند .

Introduction

Since the introduction of closed chest compressions in 1960[1], cardiopulmonary resuscitation has become the standardintervention in CPA. The ensuing five decades have seenmany advances in resuscitation techniques, including the development of Advanced Cardiac Life Support (ACLS) procedures and comprehensive postarrest care [2]. However,the survival-to-discharge rates of 15–۲۰% are little changed,and survival with good quality of life is rare [3–۱۳].

Given the low success rate and high cost of in-hospitalACLS, an accurate understanding of predictors of survivalfrom CPA could improve decision making for patients, families,healthcare providers, and healthcare payers when consideringthe appropriate application of resuscitation efforts.Factors previously shown to predict poor CPA outcomes,although with variable strength, consistency, and patientpopulations [14], include: CPA during off-hours [13, 15–۱۷],longer duration of CPR [18], delayed initiation of CPR/defibrillation [17–۱۹], comorbidities [6, 9, 12, 18, 20–۲۵],unwitnessed (versus witnessed) arrest [9, 13, 23], primarycardiac (versus primary respiratory) arrest [9, 13, 25], pulselesselectrical activity (PEA) or asystole (versus ventriculartachycardia (VT) or ventricularfibrillation (VF)) [9, 13, 16, 20,25, 26], African-American race [27], increased age [9, 16, 18,20, 23, 24, 28–۳۰], and decreasedBMI [25, 31]. There are somedata to suggest that hospital bed typemay influence outcomes[9, 12].

There has been increased use of the National Registryof Cardiopulmonary Resuscitation (NRCPR) to report outcomesand assess risk factors from a far larger and morediverse patient population that can be done in smaller studies [15, 27, 32–۳۴]. We were interested in determining whetherfactors influencing CPA outcomes were similar in our largeurban academic medical center, which features experiencedintensivists managing patients during and immediately afterarrest. We present a retrospective review of all resuscitationattempts at the UMMC from 2000 to 2005, examiningpatient demographics, arrest characteristics, comorbidities,unit and service type, and witnessed status on survival.Wehypothesized that there would be important differences inoutcomes and risk factors from our institution, with its differentpatient populations and CPA/post-CPA management,from those in the broad-based medical literature. Any significantdifferences could be critically important in decisionmaking regarding resuscitation attempts at institutionswhosepatient population differs fromthose in the aggregateNRCPRdatabase.

۳٫ Results

During the study period, 729 arrests occurred in 709 patients. Table 1 details patient demographics, admission service, and bed type for these patients. 476 (67%) were male, with an average age of 58 ± ۱۶ years. Caucasian and African Americans comprised 349 (51.8%) and 290 (43%) of patients, respectively. Most patients had a surgical or medical admitting service, with 122 (16.7%) on cardiology. The majority of arrests, 453 (62.1%), took place in an ICU.

Characteristics of arrest and the outcomes of resuscitation are shown in Table 2. The average duration of CPA was 24 ± ۲۲ minutes, and CPAs were evenly distributed throughout the day. 377 (50.8%) CPAs were cardiac, 185 (24.9%) were respiratory, and in 180 (24.3%) the type of arrest could not be determined. Most arrests were witnessed 649 (91.4%) and monitored (84.2%). 383 (53.9%) patients had a return of initial circulation following the resuscitation attempt, 150 (21.3%) survived to discharge, and 70 (9.6%) were discharged home.
